THE APPLICATION SHOULD BE COMPLETED BY THE CHILD'S PARENT OR LEGAL GUARDIAN.
ONLY ONE APPLICATION PER FAMILY WILL BE ACCEPTED.

SUBMITTING A 2ND APPLICATION MAY RESULT IN THE REMOVAL OF BOTH APPLICTIONS.

What happens next…

By mail, you will receive an appointment letter providing the Location, Day, Date and Time of your personal interview. This letter will be sent to the home address you provided.

PLEASE BE PATIENT. APPLICATIONS ARE AVAILABLE WELL IN ADVANCE OF THE INTERVIEW DATES ….IT MAY BE FOUR OR FIVE WEEKS BEFORE YOU RECEIVE YOUR APPOINTMENT LETTER.

The letter will also outline the identification documents you will need to bring with you to a personal interview with a Goodfellow Fund volunteer. Goodfellow volunteers will validate your eligibility and also the eligibility of your children.
